Partnership Forms
Donation Receipt Request -Use this form to request a receipt for any in-kind or monetary donations to your campus or department. A receipt will be emailed to you immediately. A record of each donation will be recorded into your Q-drive Partnerships Folder each month.
Partnership Agreement Request -Use this form to request a formal Partnership Agreement listing a brief description of your Partner's activity & your campus' reciprocal activity.
To record any partner activity that is less formal, and does not need a Partnership Agreement, please go to your Q-drive Partnership Folder and list the requested information in the Partnership Activity spreadsheet.
